What is the Home from Hospital Service?
Going home after a stay in hospital is not always easy. Trained British Red Cross volunteers can offer short-term support and practical assistance to help people return to their daily routines. They can complement the help which social and community health workers may provide and assist clients to regain their independence through a service that is tailored to their needs.
Tasks which their volunteers can undertake for you include: collecting prescriptions, shopping, preparing clients' homes on their return, providing companionship and confidence building.
The Service is FREE
Who is eligible?
If you live in Worcestershire the service is available to any hospital patient who needs that extra bit of support, for up to four weeks after discharge.
How to apply?
if you would like help returning home or know someone else who needs the Service.
Ask the Ward Sister or Staff Nurse to contact the Home From Hospital Co-ordinator on:

The Co-ordinator will visit you, either in hospital or at home, to make the necessary arrangements.
